6. Risk Evaluation (Before Controls)

Likelihood Rating

Score Description
1 Unlikely โ€” exposure rarely or never occurs under normal use
2 Possible โ€” exposure may occur occasionally
3 Likely โ€” exposure occurs regularly under normal conditions

Severity Rating

Score Description
1 Low โ€” minor, reversible effects; first aid only
2 Medium โ€” significant harm; medical treatment required
3 High โ€” serious, irreversible harm or death

Risk Score = Likelihood ร— Severity

Score Risk Level Action Required
1โ€“2 Low Acceptable; monitor and review
3โ€“4 Medium Improve controls where reasonably practicable
6โ€“9 High Work must not proceed until adequate controls are in place

Initial (Uncontrolled) Risk Score

Likelihood (1โ€“3) Severity (1โ€“3) Risk Score Risk Level

7. Control Measures

Feel free to discuss this below. This was majorly an ai generated template and may have some areas which are not needed or can be adjusted thoug I think is generally in track. I would maybe add the addition of the symbols for some visual identification. Also the WELโ€™s may not be fully required due to the nature of the space. (Anything that is that harmful enough to require a short term exposure limit assessment I probably would allow in the space personally.)
